[ARCHIVED] Director of Care Management (MUST BE A RN) - Montana

Responsible for the programmatic development, planning, organization and management of the functions that focus on care facilitation and movement of patients through the hospital continuum. This includes, but is not limited to: Case Management, Social Work, Utilization Management and post-acute services coordination. Develops the Hospital Case Management program to enhance the distinction in staff roles related to progression of care.

Must have a RN and a BSN. Master's degree is preferred. 3-5 years case management in a hospital setting. 2-3 years in a leadership position.

Facility is around 100 beds and census averages 75. The position reports to the CNO and CFO and oversees 7 FTE's. The leadership team is wonderful and what makes this facility a great place to work.
Looking for someone that can be a leader with a "we" attitude as they would be a team. Person needs to be a resource for the group. Experience with Bedside Case Management, Utilization Review, Discharge Planning and Denials as this is all part of what this person is overseeing. The salary range for the position is between $90,000 - $110,000 and they offer full benefits including relo and sign-on.
